I've been stung by a scorpion! Now what?

Essential Spider Bite First Response by Jonathan Leeming

First response is focused upon predicting outcomes, recognising symptoms and understanding the severity of the situation. A decision needs to be made to do nothing, or transport the patient to medical assistance as soon as possible. Between these two extremes lies the answer to the question: What should I do?

When responding to a scorpion sting event, begin by assessing the situation in 3 ways:

  • Question the patient.
  • Are there any factors that influence symptoms severity (see page 129)
  • Are there any red flags (see page 139).

Begin by first asking the following basic questions. Avoid leading questions by allowing the patient to express themselves in their own words.

  • How are you feeling? - The patient’s current symptoms indicate the most important effects of the scorpion sting. Avoid leading questions, such as “how painful is the sting?” or “are you feeling drowsy?”. Rather ask how do you feel and let the patient describe the symptoms in their own words.
  • Where have you been stung? - There may be no or very little visual evidence of a sting. Where possible make a mental note of the location of the sting.
  • Where is the scorpion now? - After stinging, the scorpion will often move away from the area or hide. If the scorpion is available, then take a photo of the scorpion for identification.
  • Can you describe the scorpion? - It is extremely helpful if the scorpion is available for identification. In some cases the scorpion was not seen at all, or it may not have even been a scorpion.
  • When and under what circumstances were you stung? - The circumstances that led up to the scorpion sting. The answer may give an indication as to the species of scorpion responsible.

If the patient is a child or animal, then it may be difficult to ask these questions and get a meaningful reply. Assessing symptoms is based upon observing and interpreting any unusual behaviour. Asking these questions in a calm and relaxed manner  reassures the patient and greatly assist in the treatment of the patient.

Factors That Influence Symptom Severity

Are there any significant factors that influence symptoms severity. How do these factors influence the outcome of the scorpion sting?
